Application Process
To register a team for SISL, follow these steps:
- Complete the ONLINE APPLICATION FORM. Please double-check the email addresses for the coach and manager. Submit the form online.
Age brackets based on USYSA age groups:
For the seasonal year beginning Sept. 1, 2009 and ending August 31, 2010
U-19 (8/01/1990 - 7/31/1991)
U-18 (8/01/1991 - 7/31/1992)
U-17 (8/01/1992 - 7/31/1993)
U-16 (8/01/1993 - 7/31/1994)
U-15 (8/01/1994 - 7/31/1995)
U-14 (8/01/1995 - 7/31/1996)
U-13 (8/01/1996 - 7/31/1997)
U-12 (8/01/1997 - 7/31/1998)
U-11 (8/01/1998 - 7/31/1999)
U-10 (8/01/1999 - 7/31/2000)
U-9 (8/01/2000 - 7/31/2001)
U-8 (8/01/2001 - 7/31/2002)
- Download and print the following items (Note - for best results, set margins-left, right, top and bottom - to 0.5". Set headers/footers to 0.0". Paper size is 8.5"x 11"):
Complete the Roster Form. Have parents fill out SISL Medical Release/Code of Conduct form. * Note: ALL players must use THIS SISL form. These need to be signed by both player and parent. Rosters and Medical Release/Code of Conduct forms will be collected at Registration Night. These must be turned in for player to play.
- Complete the online Kidsafe Application (Note - Each coach and volunteer associated with the team will need to complete the online Kidsafe Form")
- An acceptance letter will be emailed to you upon completion of your team's registration.
- A subsequent email will alert you to the posting of schedules on the website.
MANDATORY REGISTRATION NIGHT
LANDSTOWN MIDDLE SCHOOL
November 18, 2009 from 6:00 p.m. - 9:00 p.m.
Please bring the following to Registration Night:
- 2 copies of the completed ROSTER
- Medical Release/Code of Conduct forms filled out and signed by parent and player.
- Payment for the tournament and season is $700 per team. If you are interested in only playing the Frostbite Tournament the payment is $300 per team. (Only one check accepted made out to "Southside Indoor Soccer League").
